Insurance/Payment

Insurance – I am an In-Network provider for almost all insurance companies. Among these are: BCBS, Medica, Cigna, United Health Care, Preferred One, UCare, Medical Assistance, and many EAP programs, but check with your provider for assurance. I may be listed as Darlene Merchant, M.A., L.P. as well as Pathways to Change, LLC. You can find other important information about your coverage from your insurance provider (e.g., coverage for mental health, deductible amount and amount remaining if any, co-payments, authorization or approval required.) If you have other insurance, we can check to see if I am in-network or if they pay for “out of network” providers.

Fees:  First session:  $200.00, individual sessions: $140.00, family/couple: $160.00 

Payments Accepted – Insurance, HSA (Health Savings Account) , FSA (Flexible Spending Account), cash, check, and credit cards (VISA, Master Card, American Express, Discover.)

Late cancellation Policy – For appointments cancelled with less than 24 hours advanced notice or for missed appointments there will be a charge of $80.00.
